Waiver Services: No-Cost Senior In-Home Services
Alpha In-Home Services, LLC is an Illinois Department on Aging Provider for In-Home Services.
The Illinois Department on Aging's Community Care Program (CCP) offers services and supports to qualifying older Illinoisans to help work with them to remain in their homes, if that is their choice, and avoid unnecessary institutionalization.
Seniors are eligible for CCP services at no cost if they:
• Are age 60 or older;
• Are determined physically in need of services and supports based on the comprehensive assessment tool;
• Are a resident of the State of Illinois;
• Are either a U.S. citizen or an eligible non-citizen within the specific categories;
• Meet the asset requirements; and
• Agree to apply, and if found eligible, enroll for Medicaid benefits on an annual basis. Each individual is different, and may be eligible to receive different services.
There is no out of pocket cost to the qualified senior for CCP services. A Care Coordinator will be sent to your home at no cost to discuss the senior's needs and person-centered goals, and to help them choose what services may be right for them.
Determination of eligibility:
Local Care Coordination Units (CCUs) will complete an assessment to determine the senior's eligibility for services. They will ask the senior what kinds of tasks they can complete on their own, what kind of activities they require assistance with and what type of support they have available.
If the senior is determined eligible, the Care Coordinator will also work with them to identify their person-centered goals and arrange services and supports needed.

If the senior is determined eligible
If the senior qualifies for services, the Care Coordinator will work with the senior to develop a person-centered plan, set up services that are appropriate for them which they have agreed to receive and make sure these services continue to meet their needs.
In-home services
As an IL Dept. on Aging's provider, Alpha In-Home Services, LLC offers non-medical support to the eligible senior by supervised homecare aides who have received specialized training in the provision of in-home services. See here what the services include.

A senior who has been approved for services has the option to receive services from a family member or a preferred individual, otherwise known as a Family Homecare Aide. See more information here.
